(a) Referenced bulletins, instructions and forms are for use in administering grants made under this part and are available from any USDA/Rural Development office or the Rural Utilities Service, U. S. Department of Agriculture, Washington, DC 20250-1500. (b) Applicants will be required to submit the following items to the processing office, upon notification from the processing office to proceed with further development of the full application: (1) Form RD 400-1, Equal Opportunity Agreement; (2) Form RD 400-4, Assurance Agreement; (3) Form AD 1047, Certification Regarding Debarment, Suspension and other Responsibility Matters; (4) Form AD 1048, Certification regarding Debarment, Suspension, Ineligibility and Voluntary Exclusion—Lower Tier Covered Transactions; (5) Form AD 1049, Certification regarding Drug-Free Workplace Requirements (Grants) Alternative I for Grantees Other Than Individuals; (6) RUS Form 266, Compliance Assurance form or written self-certification statement—Civil Rights Compliance; (7) Standard Form LLL, Disclosure of Lobbying Activities; (8) RD Instruction 1940-Q, Exhibit A-1, Certifications for Contracts, Grants, and Loans (Regarding Lobbying); and (9) Certification regarding prohibited tying arrangements. Applicants that provide electric service must provide the Agency a certification that they will not require users of a water or waste facility financed under this part to accept electric service as a condition of receiving assistance.
