(a) (b) (1) The length of additional time required to complete project objectives and a justification for the extension; (2) A summary of progress to date (a copy of the most recent progress report is acceptable provided the information is current); and, (3) Signature of the Authorized Representative and the Principal Investigator requesting the extension. Any request received by the ADO that does not meet this requirement will be returned for the necessary signature(s). (c) (d)
